Produced for the Association of British Pharmaceutical Industries (ABPI) in 1987 to highlight what can happen when Good Manufacturing Practices are not followed. The program is divided into three vignettes.
Vignette 1: Ergometrine Production, the primary problem is the production of a batch using the wrong ingredients. This results from a series of errors that start with the shipping/receiving clerk misreading the label on the drum.
Vignette 2: The primary problem is the production of a batch using contaminated equipment. This results from a series of errors starting with an operator who has a toothache and shortcuts the cleaning process.
Vignette 3: Tabrim Production, we see a mistake in packaging and labeling, one of the most common reasons for a recall. In this vignette, the drug Tabrim, used in treating severe pain, is being made in doses of 2.5, 10 and 25 milligrams.
